This service is only for an individual who wishes to come to the appointment alone.  This initial visit lasts fifty minutes on average.  Please arrive 10 minutes before the start of the appointment in order to fill out the 3-page patient information form which will be attached to a clipboard in the waiting room.  Please bring your insurance card(s) with you.  FSA will submit your insurance claim for you and the total out-of-pocket cost to you will be the CO-PAY that is assigned by the insurance company.
