This service is only for a parent(s) who wishes to bring a child to the therapy session..  This initial visit lasts fifty minutes on average.  Please arrive 10 minutes before the start of the appointment in order to fill out the 3-page patient information form which will be attached to a clipboard in the waiting room.  Please bring your insurance card(s) with you.  FSA will submit your insurance claim for you and the total out-of-pocket cost to you will be the CO-PAY that is assigned by the insurance company.
